To support a fair and accurate evaluation process, each Official Nominee is asked to complete a brief professional profile. This information helps verify eligibility, confirm appropriate award placement, and assist the independent awards committee in its review.

Only one nomination may be submitted per entry form. Individuals and teams may be nominated by themselves or by others. All submitted materials are handled with discretion and used solely for evaluation and recognition purposes.
