To support a fair and accurate evaluation process, each Official Nominee is asked to complete a brief professional profile. This information helps verify eligibility, confirm appropriate award placement, and assist the independent awards committee in its review.

Only one nomination may be submitted per entry form. Individuals may nominate themselves or be nominated by another person, company, client, colleague, or professional organization.
